The attached template can be used for submitting Case Studies to BrainMaster for reporting and publications.  This will provide a uniform format for publications that compile multiple case studies.

The second file was modified by Kim Chmura on behalf of the Neurofeedback Association of Mid America (NAMA).  It is more complete, so we recommend its use.
